STA515W fault, any suggestion?

Hi dear, I'm trying to set up an audio amplifier with quad half bridge STA515W.

I've attached the schematic; is there anything I'm making wrong?

Every time I solder in my custom PCB it blows up immediately.

Please note that the circuit was designed for STA516B, so Vcc should be 50v, but now with STA515W is 24v.
