    Your memory map looks fine and chip select equations for CSBOOTx looked fine also. Please take a look at the attached sample code on EEPROM Emulation, it has samiliar kind of memory mapping scheme, i.e. Main Flash mapped to Program Space which contains code to program the Secondary Flash CSBOOTx.
