What can possibly cause USBLC6-2P6 failure?

In our application USBLC6-2P6 is connected between a USB port and a processor STM32L4R5xx. The circuit is self-powered (Li-ion battery), and uses either PC USB or USB dc power supply for charging and communication. We find a number of damaged USBLC6-2P6 with a specific failure pattern: both low side diodes show 0.8V voltage drop in both direction, while both high-side diodes and a Vbus TVS diode are open (see image below). What can possibly cause these failures? Are there any test reports for this device and what are the typical failure modes?
