Lacking any contextual detail.

Probably conflicting with prehistoric firmware on the ST-LINK, perhaps try updating that first.

Some custom board? Could perhaps have issues with flash if power supply inadequate.

yes custom board, power supply should be enough , after we replaced a MCU , then no issue

only one out of 300 happened
