My design with SPV1040T doesn't work Any ideas why ?

Hi ST communitiy,

Here is the schematics of my design using SPV1040T chip. I used the eDesign tools to generate this schematics. The jumpers JP1 and JP2 has been closed.

I just burned the chip while applying 2V 1.4A with a laboratory source. The battery is a 12V lithium battery with its own internal security circuit.

I thought the chip was supposed to self protect itself in case of over current or overheating.

Any clue of what could have append ?
